Vineland 3 Adaptive Functioning Assessment. A complete, paced, trauma-informed adaptive functioning assessment using the Vineland-3. Used most often for Colorado Long-Term Care Medicaid Waiver eligibility.
One flat rate of $450 includes the caregiver interview, Vineland-3 scoring and analysis, a written clinical report, and a submission-ready format for your Single Entry Point, Case Management Agency, attorney, or court. $100 additional fee for expedited report.
