Get in touch
Let’s talk.
Families, attorneys, case managers, clinicians, and organizations all reach me through this page. Whatever brings you here, tell me what you are working on, and I will help you figure out the right next step.
Most common reason people reach out
Adaptive Functioning assessments for Long-Term Care Medicaid Waivers
Colorado requires an IQ or Adaptive Functioning assessment completed within the last three years to qualify for most Long-Term Care Medicaid Waivers. If your previous assessment is aging out, or you have never had one done, I can help with that.
- A free 15-minute call so you can ask questions, and so I can make sure I am the right fit before anyone commits to anything.
- An assessment process paced respectfully for the person being assessed. No rushing, no pressure.
- A complete, formatted report ready to submit to your Single Entry Point or Case Management Agency.
- Clear timelines, so you know exactly when the finished report will be in your hands.
Other reasons to reach out
I also work with attorneys, agencies, clinicians, and individuals navigating other systems that require clinical documentation. Disability, immigration and asylum, adoption and foster care, and workplace evaluations among them. The dropdown in the form helps me route your message to the right place.
I read everything that comes through. If I am not the right person for what you need, I will tell you so and try to point you toward someone who is.
